1、RESEARCH ETHICS REVIEW CHECKLISTRESEARCH ETHICS REVIEW CHECKLISTThis checklist should be completed for every research project that involves human participants,personal,medical or otherwise sensitive data or methodologically controversial approaches.It isused to identify whether a full application fo

2、r ethics approval needs to be submitted.The researchethics review process is not designed to assess the merits of the research in question,but is merely adevice to ensure that external risks have been fully considered and that an acceptable researchmethodology has been applied.This checklist applies

3、 to research undertaken by both staff andstudents,but it should be noted that the way the checklist is processed differs between these twogroups.ForFor staffstaff:if a full application is required please ensure that you complete the Ethics ReviewQuestionnaire for Researchers and send the completed f

4、orm to Michael Nelson in the ResearchDivision(RD).Please accompany the questionnaire with a copy of this checklist and a copy of the researchproposal.For MSc/PhD studentsFor MSc/PhD students:if a full application is required please ensure that you complete the EthicsReview Questionnaire for Research

5、ers and discuss the issues raised with your student supervisor inthe first instance.You should ensure that the completed forms are accompanied with a copy of theresearch proposal to ensure that your supervisor can make a fully informed decision on the ethicalimplications of the research.Where the su

6、pervisor is satisfied that all ethical concerns have beenaddressed s/he must sign the checklist and ensure that a copy is retained within the department as arecord of the decision reached.It is appreciated that in certain cases the student supervisor may notbe able to reach a decision on the ethical

7、 concerns raised.In such instances the matter should bereferred to the Research Ethics Committee(please send all relevant forms and a copy of theproposal to Michael Nelson in RD).Only where an informed decision cannot be reached by thesupervisor should paperwork be submitted to the Research Ethics C

8、ommittee.ForFor undergraduateundergraduate studentsstudents:After completing the checklist,undergraduate students shoulddiscuss any issues raised with their supervisor in the first instance.If fully satisfied with theresearch proposal,the supervisor can sign the checklist on behalf of the department

9、.A copy of thesigned form should be retained by the department as a record of the decision reached.It isappreciated that in certain instances the student supervisor may not be able to reach a decision onthe ethical concerns raised.In such instances the application for ethics approval should be refer

10、redto the Research Ethics Committee(please send all relevant forms and a copy of the proposal toMichael Nelson in RD).Only where an informed decision cannot be reached by the supervisorshould paperwork be submitted to the Research Ethics Committee.BeforeBefore completingcompleting thisthis form,form
