1、拱顶冷热熔盐储罐焊接施工HSE保证措施 12 HSE Assurance Measures12.1HSE安全保证措施12.1HSE safety assurance measures12.1.1 吊装安全措施。现场吊装作业必须由一名起重工统一指挥。在吊装转移过程中，起重人员需确保吊装机械的安全可靠、周边人员机械设备的安全、物品的安全等。做到“十不吊”：斜吊不吊；超载不吊；散装物装得太满或捆扎不牢不吊；指挥信号不明不吊；吊物边缘锋利无防护措施不吊；吊物上站人不吊；埋在地下的工件不吊；安全装置失灵不吊；光线阴暗看不清吊物不吊；吊物下站人不吊；12.1.1 Lifting Security Meas

2、ures The field lifting works must be unified commanded by one rigger. In the transfer process of lifting, lifting personnel need to ensure that the lifting machine is safe and reliable, ensure the safety of perimeter personnel, machine, devices and other items. 10 NOs should be followed: NO oblique

3、lifting; NO overloading; NO lifting when bulks is overfilled or binding is not strong; NO lifting when command signal is not clear; NO lifting when without protective measures for sharp edge hanging items; NO lifting when people standing on the hanging objects; NO lifting for buried objects; NO lift

4、ing when safety device failure; NO lifting when it is too dark to see the hanging objects; NO lifting when people standing under handing objects.12.1.2高空作业安全要求。对高空作业人员必须定期检查身体，对有高血压、心脏病等不适宜进行高空作业人员，严禁高空作业。另外，高空作业人员必须戴好安全帽，系好安全带，配齐相关安全保护装备，并掌握其使用方法。12.1.2 Safety requirement for working at height For

6、 must wear helmet, safety belt, be equipped with relevant safety equipment, and master their use.12.1.3临时用电和动火作业安全。现场临时用电必须由专职电工进行接线、管理，严禁其它人员私自乱接，严禁任何人带电操作或带负荷接电断电；配电箱的漏电保护器试跳应动作灵敏完好，开关闸刀的熔断器禁止用铜丝铁丝等代替；严格执行“一机、一闸、一漏的规定，不得将一个开关同时接两台以上的电器设备；用电设备都应做保护接零、接地；插座插头应完好，不得有开裂和缺损，禁止用电线直接插入插座内。电焊机的焊把线应采用橡胶护套软

7、线，焊钳应牢固，接头应用绝缘胶布包好防止漏电。气焊工的氧气带、乙炔带通气应良好，不应有漏气的现象，乙炔表应装有阻火器，工作面应清理可燃物，有火源的场地，氧气瓶、乙炔瓶的摆放距离不得小于10米，气瓶不能在太阳底下曝晒。氧气瓶、乙炔瓶的防护棚应分开摆设。每个用火点都必须配备2台灭火器，动火时必须设有监火人。手提电动工具（如砂轮机、磨光机等）的电源线不得有破皮漏电，插座插头应匹配，其使用应符合安全要求。进入罐内的电缆线、焊把线必须用胶皮包裹。12.1.3 Safety for temporary power & hot work Field temporary electrical wiring a

8、nd management must be done by professional electricians, other personnel is strictly prohibited to do without permission. Anyone is prohibited to work with hot line or make power on/off with load. The distribution box leakage protection trial jump should be intact, with quick action. It is prohibit

9、to use copper or iron wire to replace the fuse of switch blade. The rule of one equipment one GFCI shall be strictly implemented, and one switch is not allowed to connect at the same time more than two electrical equipment. Electrical equipment should be grounding and connecting to neutral protected

10、. Sockets and plugs should be intact, without cracks and defects, prohibited to use wire directly inserted into the socket. Rubber sheathed cable should be used for the electrode holder wire of electric welding machine, welding tongs should be solid, the joint should be wrapped with insulating tape

11、to prevent power leakage. The oxygen hose and acetylene hose for gas welders should be in good condition, without leakage phenomenon. Acetylene meter should be equipped with flame arrester. The combustibles in the working place should be cleaned. For the venues with source of ignition, oxygen cylind

12、ers and acetylene cylinders should be kept not less than 10m, the cylinders are not allowed to expose in sunshine. The protective sheds of oxygen cylinders and acetylene cylinders should be set separately. Each fire point must be equipped with two fire extinguishers, and watchman must be provided fo

13、r hot work. The power line of portable power tools (such as grinders, sanders, etc.) shall not be damaged to allow leakage, the plug and socket should match, and their use should meet the safety requirements. The power cable and electrode holder wire that goes into the tank must be wrapped with rubb

14、er.12.2 风险识别 12.2 Risk identification12.2.1 根据公司HSE管理程序，在项目施工前，施工单位组织有关班组和相关人员对施工过程中可能产生的危害进行调查和识别，采用分析讨论会的形式针对施工的各道工序、作业场所、人员的健康安全和环境保护方面进行危害识别。12.2.1 According to company HSE management procedures, before the project construction, the construction unit shall organize relevant teams and personnel t

15、o investigate and identify the potential hazard that may generate from construction process, and identify the hazards in construction of various procedures, workplace, personnel health and safety and environmental protection aspects in the form of analysis discussion.12.2.2施工项目危害识别的施工风险主要内容有（不限于）：砂轮

19、tion measures for lifting & hoistinga.吊装前进行技术方案和安全注意事项交底，并按方案和交底严格实施。a. Technical programs and safety precautions shall be talked over in details before lifting, and be strictly followed.b.凡参加起重施工的作业人员，应取得“特种设备作业人员资格证”和“特种作业人员资格证”的证书，无证者不能上岗作业。b.The workers who participate in lifting work should obt

20、ain special equipment operator certificate and special operations personnel qualification certificate, the ones without certificates are not allowed to start the work.c.吊装准备工作完成并经检查合格，办理吊装作业许可证后方可进行吊装作业。c. Hoisting hoisting operation can be carried out after the completion of preparations and after

21、passing the examination, for lifting operations licensed.起重作业人员应熟悉此施工方案、起重设备性能、操作规程，做到信号传递准确，协调一致。d. Lifting operators should be familiar with this construction program, lifting equipment performance, operating procedures, and be accurate and consistent in signal transmission.e.吊装所用机索具、材料应具有出厂合格证；手拉

22、葫芦（倒链）使用前，应检查倒链条有无裂纹、磨损，吊钩、链条轮盘、倒卡等有无变形和扭曲，链条磨损达到15者，严禁使用。受力的上下两钩，应在同一条直线上，不得有扭弯或蹩劲等现象。在新导链使用前，必须对导链出厂合格证进行检查，无合格证的倒链严禁使用。临时用的定位块，工卡具等焊件，必须焊牢，严防焊缝开裂，至使倒链、焊件等崩出伤人。e. The device, rig and materials used in lifting should be provided with ex-factory certificate; before the use of chain block (chain hois

23、t), the chain should be checked for cracks and wear, and hook, chain wheel should be checked if any distortion or twist. Those chains that are 15% worn are strictly prohibited. The stressed upper and lower hooks should be on the same line, without twisting or bending. Before using a new leading chai

24、n, the ex-factory certificate must be checked, the chain without certificated is strictly prohibited. The weldment of positioning block, tools and clamps which are temporarily used , must be firmly welded and prevented from cracking, which may lead to chain block and welding pieces outburst for inju

25、ry.f.吊装前应全面检查方案的执行情况，确认无误后，方可作业。f.Before lifting, there should be a comprehensive inspection of the implementation of the program. The job can only be carried out after checking and confirming.g.吊起的构件不得在空中长时间停留；雨天、风力大于5级、环境温度低于-25、视线不清不得吊装作业。g. The lifting member shall not stay long in the air. Lift

26、ing is not allowed when in rainy days, the wind is greater than grade-5, ambient temperature is below -25 , or blurring of the sight.i.施工人员要正确佩戴劳动保护用具，各工种必须紧密配合，按安全操作规程施工。i. Construction workers should wear proper protective equipment; each disciplinary workers must work closely with each other and

27、carry out the construction according to the safety rules.j.作业前必须对起重工具进行检查，保证处于良好状态；吊装前应进行试吊，严格检查机械及索具的装设情况，确认符合方案要求，然后进行正式吊装；起吊要缓慢平稳，栓挂溜绳；拆卸吊装绳扣时，施工人员要防止夹、砸伤。如机索具需要代用，必须经过有关人员确认无误后，方可代用，并做好相应的记录。j.The lifting tools must be checked before working to ensure in good condition. Before lifting, trial lif

28、ting should be carried out, mechanic and rig status should be strictly checked to confirm compliance with program requirements, and then formal lifting can be performed; lifting should be slow and smooth, hanging tag line; when removing lifting rope, construction personnel shall prevent from being i

29、njured. If machine and rigging requires replacement, it must be verified by concerned personnel first, and appropriate records shall be made.k.吊车站位及压脚铺设处必须平整并用道木或跑板垫实后方可作业；构件回落动作应缓慢。k. Crane stations and presser foot must be flat and padded with woods before working; the retreating movement of struc

30、tural member should be slow.l.在吊装过程中，遵守现场秩序，服从命令听指挥，不得擅自离开本岗位。l.In the lifting process, it must follow the rules, obey orders and commands, and leaving the duty without permission is not allowed.m.尽量避免夜间进行吊装作业，如果不能避免必须安装临时的照明，有充足的照明。m.Avoid lifting work at night, if it cannot be avoided, temporary l

31、ighting must be provided, with adequate lighting.n.设专人负责起吊机械设备和电器设备的维修工作。n. Specially-assigned person shall be put in charge of maintenance of lifting equipment and electrical equipment.o.必须使用经测试合格的设备，严禁使用已损坏或有缺陷的起重设备。o. Use only tested devices; prohibit the use of damaged or defective lifting equip

32、ment.p.清理吊车作业范围内现场，设置警戒区域和警戒标志，非作业人员不得在此范围内站立和工作。p.Cleanup the site within the range of crane working, set the alert area and warning signs, non-operating personnel are not allowed to stand in this area or work.q.严禁超负荷使用吊装用机、索具。q. It is prohibited to use lifting device or rig overloaded.r.调整好吊钩在重物上的

33、起吊点，以免吊起时重物摆动。r. Adjust the lifting point of hook on the heavy objects, so as not to swing when lifting weights.s.起重钢丝绳不断股、不套扣、不缠绕、不在钢丝绳上打结来缩短绳长；s. Lifting rope shall not be damaged, knotted or twined. It is not allowed to tie knots to shorten the length of the rope. 12.2.4.2高处坠落风险消减措施12.2.4.2 Risk

34、of falling from height elimination measuresa.作业人员身体经检查合格，登高作业人员必须持有登高作业证。a. Physical examination of the workers shall be qualified; workers of working at height must get the working permit first.b.作业人员着装符合工作要求。b.The workers dress shall meet the job requirements.c.作业人员正确佩戴安全帽、安全带，对2m以上的作业安全带必须挂牢方可作业，

35、禁止安全带低挂高用；安全带挂在作业处上方牢固的构件上，不得系挂在有尖锐棱角的部位，安全带系挂点下方应有足够的净空。c.Worker should wear proper helmet and safety harness. For jobs more than 2m, safety harness must be firmly hung before working, and it is prohibited to hang in lower place for protection. The safety harness should be hung on the solid members

36、 above the working place and cannot be hung in places with sharp edge; there should be adequate clearance below the safety harness hanging place.d.作业点下方设警戒区，并有警戒标志。d.Guard zone with warning signs shall be set below the operation place.e.攀登或作业时要手抓牢、脚登稳，避免滑跌，重心失稳。e.When climbing or working, the hand o

37、r foot should hold firmly to avoid slipping or collapse.f.现场搭设的脚手架、活动平台、梯子、防护围栏必须由起重班长、使用方班长办理交接手续，工长、安全员确认：不得有探头板、绑扎牢固、安全可靠。f.Field erected scaffolding, movable platform, ladder and guard rail must be proceed hand over procedure by lifting leader and user leader and confirmed by foreman and HSE off

It should be free of protruded plate, solid binded, safe and reliable.g.尽量避免夜间高空作业；如有夜间作业要有足够的照明，必要时安装临时照明灯具。g.Try to avoid high-altitude operations at night; if working at night, it should have adequate lighting, temporary lights can be fixed, if necessary.h.严禁使用吊车、卷扬机运送作业人员。h.It is prohibite

39、d to use crane or windlass to transport operation worker.i.物体坠落伤害的风险消减措施i.The risk of falling objects damage elimination measuresj.现场设专人监护，及时清理高处附着物，检查构件是否松动。j.Assign someone to keep watch at site, clean the fixtures at height, checking if members are loose.k.作业区域下方设围栏警戒，禁止区域内有人走动。k.Fencing shall be

40、 set below the working area, forbidding people walking in this area.l.作业人员正确佩戴安全帽。l.The workers shall wear safety helmet properly.m.施工人员配备工具袋，施工工具和工件等有防滑落措施；m.Tool bag is provided for workers, with anti-slip measures for construction tools and workpiece.n.不得上下交叉作业，如有分层作业，中间应有可靠的隔离措施，必要时设安全网。n.Cross-

41、operation up and down is not allowed, if working in different layers, there should be reliable isolation measures in the intermediate, provided a safety net if necessary.o.严禁向下抛掷物体。o.It is strictly prohibited to throw objects.p.采取正确的施工方法，作业人员不得有侥幸心理。p.Taking the right construction methods, workers s

44、resa.放射探伤人员必须具有有合格的放射线操作资质。a. Radiation inspection personnel must have qualified radiation operation qualification.b.射线作业严禁白天进行，如因特殊需要必须白天作业时，要有严格的安全防护措施，必须经安全环保部门审批方可作业。b.Ray operation is prohibited during the daytime, if it must be carried out during the day for special needs, there must be strict

45、 security protective measures and must be approved by safety and environmental protection sectors before the job.c.射线探伤人员应配戴好符合标准的防护用品，作业时携带好个人剂量的检测仪器。c.Ray detection operation personnel should wear qualified protective equipment, and bring inspection device of personal dose meter when working.d.射线作

46、业时通知安全环保部门和有关单位，经同意后方可在经批准的区域内进行工作，作业时要在危险区外设好时设警戒线，并在主要道路或通道上设专人监护，作业时间应在规定时间内进行。d.Safety and environmental protection department and relevant units should be notified of ray detection work, which should be approved before carrying out in the agreed region. 48、der should wear canvas uniform to prevent arc burns.12.2.4.5有关气体使用的风险消减措施 12.2.4.5 Risk of gas use elimination measuresa.当运输、存贮和使用时，应避免剧烈的震动、冲击和直接爆晒，气瓶帽应保护完好。a.When transport, store and use, it should avoid severe vibration, shock or direct exposure in sunlight. Cylinder cap should be well preserved

49、.b.为防止空气进入气瓶，气瓶内气体不得用完；b.In order to prevent air going into the cylinder, the gas in the cylinder can not be wholly used;c.按种类分别直立存放；氧气瓶与乙炔瓶的安全距离不得小于5m，氧气瓶和乙炔瓶与动火地点的安全距离不得小于10m；氧气瓶不得曝晒，乙炔瓶不能倒放。c.It shall be stored upright in types respectively; a safe distance from oxygen cylinders and acetylene cylinders must not be less than 5m,
